New patients/registering with us

The doctors welcome new patients who live within our practice area.

When you move into the Practice area, you can visit the Surgery and fill out a Patient Registration form at Reception. When this is signed by one of the Doctors, you have been accepted by the Surgery. We will ask for proof of identity, preferably photographic proof such as a driving licence or passport, and also proof of your address.

We will also ask you to fill out a simple health questionnaire. This is to help us to care for you until your full medical records arrive at the Practice.

Every patient at the Practice is registered with the Practice as a whole and has one of the Practice GP as their named, accountable Doctor. You have a choice of GP when you visit the Practice and this can be any Doctor, not necessarily your named GP.

Every new patient should make an appointment with the Nurse for a New Patient Health Check.
